Process and Procedures to Work with Us
- All services will require the completion of the appropriate forms (all listed) located under Forms. We utilize Carepatron, a healthcare platform, that allows you to book services, complete paperwork, send payment, and more. You will be sent an email with instructions and links to complete appropriate forms.
Specific to Veterans Services
- For Mental Health Evaluations, these are for those who suspect a mental health diagnosis that is service connected, but may not have been previously diagnosed, a secondary condition has developed, a condition has become aggravated, or the possibility of increasing claims percentages. Mental Health Evaluations require an interview (conducted remotely via a secure telehealth platform) in addition to completion of forms, getting us appropriate medical records, and paying fees.
- For DBQ's and/or Nexus Letters, these are for pre-existing mental health conditions that are service connected. These will not require an interview and will only require you to complete appropriate forms, get us your medical records, and pay your fees. Email us at drabutler@a-n-telepsych-assessment-associates.com in order to begin this process.
- For Rebuttal Letters, these are for current clients in order to address inaccurate or incorrect statements in a denial letter received from the VA. These may or may not require an interview, completion of appropriate forms, obtaining any additional medical records, and paying fees.
